Maison McCulloch Hospice (MMH) is a Centre of Excellence dedicated to the collaborative delivery of quality hospice palliative care in both official languages. POSITION SUMMARY

The RPN provides direct nursing care to residents in accordance with established policies and procedures of MMH including:  assessing and anticipating the total physical, emotional, spiritual, and psycho-social needs; planning, documenting, and implementing care plans; and evaluating effectiveness of care plans and making recommendations for changes where necessary. The RPN assumes the care of those residents who are stable and have predictable outcomes and assists the Registered Nurse (RN) with the care of those residents who are unstable and have unpredictable outcomes. RPNs are expected to be positive role models for staff, volunteers, guests, family members, health care professionals, and the community.

POSITION REQUIREMENTS

Necessary:

  • Current license with the College of Nurses of Ontario
  • Certified in Learning Essentials Approaches to Palliative Care (LEAP Core) within 3 months of hire
  • Police Vulnerable Sector Check
  • Demonstrated understanding of pain and symptom management
  • Certified in medication administration

 

Preferred:

  • Recent medical / Hospice Palliative Care (HPC) experience is an asset
  • Fundamentals & Comprehensive Advanced Palliative Care Education (CAPCE) if applicable
  • Bilingualism (English/French), written and verbal, (or willingness to pursue) is an asset
  • Certified in IV administration an asset

Other Skills / Abilities:

  • Knowledgeable about, and committed to, the Hospice philosophy of care
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to organize and prioritize work.

SALARY: Scale from $31.30 per hour

REGISTERED NURSE (RN)

CLINICAL MANAGER – RESIDENTIAL PROGRAM, Sudbury ON

NAVIGATOR – COMMUNITY HOSPICE PALLIATIVE CARE TEAM

Part time, one year, contract position,

22.5 hours bi-weekly – 7.5-hour per day,  alternating Thursdays and every Friday. Must be available to work statutory holidays.

POSITION SUMMARY

The Navigator is responsible for providing administrative and clerical support for the Community Hospice Palliative Care Team.

MAJOR AREAS OF RESPONSIBILITY

  • Communicates with both internal and external stake holders as to Community Hospice Palliative Care Team activities.
  • Responds to all general enquiries in a timely manner.
  • Ensure proper technical support and troubleshoots when necessary.
  • Manages all data relating to the program.
  • Assures confidentiality of the program.
  • Coordinates all the logistics related to service operations.
  • Controls all inventory for the Community Hospice Palliative Care Team.
  • Performs other job-related responsibilities and duties as assigned.

POSITION REQUIREMENTS

Necessary:

  • Clear criminal record.
  • Fluency in both (oral and written) French & English languages.

 Preferred:

  • Successful completion of a business administration or other related diploma or degree from a college or university.
  • Experience in hospice palliative care.

 Experience / Skills:

  • Minimum of 3 years of experience in an administrative function
  • Competency in Microsoft applications including Word, Excel, Outlook and Adobe
  • Keyboarding skills of 40+ wpm.
  • Excellent verbal and written communication and interpersonal skills.
  • Strong organizational skills with attention to details.
  • Ability to multi-task and to work well in a fast-paced environment.

COMMUNITY HOSPICE PALLIATIVE CARE NURSE (RN)

Part-time Position (averaging 30 hours / 4 shifts bi-weekly)

Working alternate Monday & Thursday (8:30 am to 4:30 pm)

& every second weekend (Saturday & Sunday 8:30 am to 4:30 pm)

POSITION SUMMARY

As part of the Hospice Palliative Care Team (HPCT), the Registered Nurse is responsible for providing telephone support / consultation to clients that have a life limiting illness and are in the last year of life, and their caregivers.

MAJOR AREAS OF RESPONSIBILITY

  • Provides comprehensive telephone assessments with a holistic approach, to address pain and symptoms experienced during multiple illness trajectories of life limiting illness
  • Performs assessments, care plan development and ongoing monitoring of each client in accordance with the established Maison McCulloch Hospice (MMH) Policies and Procedures
  • Reports all findings to the assigned Most Responsible Provider (MRP) whether this is the Physician or Nurse Practitioner
  • Schedules home visits for these providers so that client’s ongoing needs are met
  • Collaborates with the North East Cancer Centre, “Ontario Health atHome” and their contracted nursing providers

POSITION REQUIREMENTS

Necessary:

  • BScN in Nursing with a certificate of competence with the College of Nurses of Ontario
  • Minimum of 5 years full-time equivalent nursing experience
  • Ability to communicate effectively with clients and family in person or via telephone and troubleshoot any of their concerns
  • Receptive to ongoing education in Hospice Palliative Care
  • Must have valid driver’s license and access to a vehicle
  • Clear Vulnerable Person Check

 Preferred:

  • Recent minimum of 2 years full-time equivalent in Hospice Palliative Care (HPC) experience
  • Certified or receptive toward achieving certification in Hospice Palliative Care with the Canadian Nurses Association (CHPCN(C) designation)
  • Proficiency in French, (verbal and written)
  • Experience providing telephone nursing in a sensitive and compassionate manner
  • Ability to develop a plan of care with clients, primary caregivers and community providers
  • Strong knowledge of community resources and ability to refer as needed for various community services

 Personal Skills:

  • Demonstrates at all times clinical autonomy
  • Working knowledge of computer software applications Microsoft Office, (Excel, Word, Outlook)

  • Performs assessments, care plan development and ongoing monitoring of each client in accordance with the established Maison McCulloch Hospice (MMH) Policies and Procedures
  • Reports all findings to the assigned Most Responsible Provider (MRP) whether this is the Physician or Nurse Practitioner
  • Schedules home visits for these providers so that client’s ongoing needs are met
  • Collaborates with the North East Cancer Centre, “Ontario Health atHome” and their contracted nursing providers

POSITION REQUIREMENTS

Necessary:

  • BScN in Nursing with a certificate of competence with the College of Nurses of Ontario
  • Minimum of 5 years full-time equivalent nursing experience
  • Ability to communicate effectively with clients and family in person or via telephone and troubleshoot any of their concerns
  • Receptive to ongoing education in Hospice Palliative Care
  • Must have valid driver’s license and access to a vehicle
  • Clear Vulnerable Person Check

 Preferred:

  • Recent minimum of 2 years full-time equivalent in Hospice Palliative Care (HPC) experience
  • Certified or receptive toward achieving certification in Hospice Palliative Care with the Canadian Nurses Association (CHPCN(C) designation)
  • Proficiency in French, (verbal and written)
  • Experience providing telephone nursing in a sensitive and compassionate manner
  • Ability to develop a plan of care with clients, primary caregivers and community providers
  • Strong knowledge of community resources and ability to refer as needed for various community services

 Personal Skills:

  • Demonstrates at all times clinical autonomy
  • Working knowledge of computer software applications Microsoft Office, (Excel, Word, Outlook)
  • Self-directed with exceptional organizational and interpersonal skills
  • Professionalism in all work settings (community in home setting and Maison McCulloch Hospice)

POSITION SUMMARY

As part of the Hospice Palliative Care Team (HPCT), the Registered Nurse is responsible for providing telephone support / consultation to clients that have a life limiting illness and are in the last year of life, and their caregivers.

MAJOR AREAS OF RESPONSIBILITY

  • Provides comprehensive telephone assessments with a holistic approach, to address pain and symptoms experienced during multiple illness trajectories of life limiting illness

  • Ensure proper technical support and troubleshoots when necessary.

  • Coordinates all the logistics related to service operations.
  • Controls all inventory for the Community Hospice Palliative Care Team.
