COMMUNITY HOSPICE PALLIATIVE CARE NURSE (RN) (designated bilingual position)

Maison McCulloch Hospice (MMH) is a Centre of Excellence dedicated to the collaborative delivery of quality hospice palliative care in both official languages. We are growing and looking to hire interested candidates who want to pursue their career in a wonderful community.

POSITION SUMMARY

As part of the Community Hospice Palliative Care Team (CHPCT), the Registered Nurse is responsible for providing telephone support / consultation to clients that have a life limiting illness and are in the last year of life, and their caregivers.

MAJOR AREAS OF RESPONSIBILITY

  • Provides comprehensive telephone assessments with a holistic approach, to address pain and symptoms experienced during multiple illness trajectories of life limiting illness
  • Performs assessments, care plan development and ongoing monitoring of each client in accordance with the established Maison McCulloch Hospice (MMH) Policies and Procedures
  • Reports all findings to the assigned Most Responsible Provider (MRP) whether this is the Physician or Nurse Practitioner
  • Schedules home visits for these providers so that client’s ongoing needs are met
  • Collaborate with the Shirley & Jim Fielding Northeast Cancer Centre, Ontario Health at Home and their contracted nursing providers
  • Supports MMH Nursing Staff with direct resident care when required to take on a daily resident assignment (ex: fill in on the residential floor during a staffing shortage, Monday to Friday 8:30 am to 4:30 pm)

POSITION REQUIREMENTS

Necessary:

  • BScN in Nursing with a certificate of competence with the College of Nurses of Ontario
  • Proficiency in spoken and written bilingual communication – French/English is required
  • Recent minimum of 2 years full-time equivalent in Hospice Palliative Care (HPC) experience with 3 years of general nursing
  • Ability to communicate effectively with clients and family in person or via telephone and troubleshoot any of their concerns
  • Receptive to ongoing education in Hospice Palliative Care
  • Must have valid driver’s license and access to a vehicle
  • Clear Vulnerable Person Check

Preferred:

  • Certified or receptive toward achieving certification in Hospice Palliative Care with the Canadian Nurses Association (CHPCN(C) designation)
  • Experience providing telephone nursing in a sensitive and compassionate manner
  • Ability to develop a plan of care with clients, primary caregivers and community providers
  • Strong knowledge of community resources and ability to refer as needed for various community services

Personal Skills:

  • Demonstrates at all times clinical autonomy
  • Working knowledge of computer software applications Microsoft Office, (Excel, Word, Outlook)
  • Self-directed with exceptional organizational and interpersonal skills
  • Professionalism in all work settings (community in home setting and Maison McCulloch Hospice)
  • Ability to maintain confidentiality when sharing and discussing personal client information with the inter-professional team

SALARY: Range from $37.46 to $54.05 (commensurate with experience)

ENVIRONMENTAL ATTENDANT

Permanent part-time, 67.50 hours every two weeks, 10-6 daily

Monday to Friday first week, Tuesday to Friday second wee

POSITION SUMMARY

The Environmental Attendant performs a variety of housekeeping / janitorial duties that maintains a clean and safe work environment for the Residents, Clients, and Staff at Maison McCulloch Hospice (MMH).

 

MAJOR AREAS OF RESPONSIBILITY

  • Provides an internal environment that maintains a safe and sanitary space for all.
  • Maintains MMH’s external space as a safe and presentable environment for all.
  • Safely operates and maintains various cleaning equipment.
  • Comprehends and applies quality practices at MMH.
  • Performs other job-related responsibilities and duties as assigned.

 

POSITION REQUIREMENTS

Necessary:

  • Clear criminal record
  • Secondary School Diploma or equivalent
  • Must possess or acquire WHMIS certification
  • Valid Driver’s License

Preferred:

  • Experience in a related position an asset
  • Fluency in English & French an asset

Experience / Skills:

  • Physical ability to perform the duties of the assigned position
  • Ability to lift heavy objects (up to 15 kg.)
  • Ability to stand for extended periods of time
  • Must be able to perform repetitive bending and reaching
  • Demonstrated skill in meeting deadlines and managing workload
  • Positive attendance record
  • Strong organizational skills with attention to details
  • Ability to work effectively in a team environment

SALARY: Starting at $21.28 to $22.85 (commensurate with experience)
