POSITION SUMMARY

The Registered Nurse (RN) provides leadership and delivers expert, compassionate palliative care to residents in accordance with MMH policies and procedures. The RN assesses and anticipates physical, emotional, spiritual, and psychosocial needs; develops, implements, and evaluates individualized care plans; and ensures compliance with medication administration and control regulations. As a clinical role model, the RN fosters a culture of excellence among staff, volunteers, families, and community partners.

Necessary:

  • Bachelor of Science in Nursing (BScN)
  • Certified Hospice Palliative Care Nurse [CHPCN(C)] or willingness to obtain within 12 months of hire
  • Current registration in good standing with the College of Nurses of Ontario
  • Certified in LEAP Core (Learning Essential Approaches to Palliative Care) or willingness to complete within 3 months of hire
  • Police Vulnerable Sector Check (current)
  • Demonstrated expertise in pain and symptom management for advanced-illness patients

Preferred:

  • Recent clinical experience in hospice/palliative care (HPC) is preferred
  • Bilingualism (English/French), written and verbal, (or willingness to pursue) is an asset
  • Certification in IV administration an asset

Essentielles:

  • Baccalauréat en sciences infirmières (B.Sc.Inf.)
  • Certification d’infirmier(ère) en soins palliatifs [CHPCN(C)] ou volonté de l’obtenir dans les 12 mois suivant l’embauche
  • Inscription en règle auprès de l’Ordre des infirmières et infirmiers de l’Ontario

NAVIGATOR – COMMUNITY HOSPICE PALLIATIVE CARE TEAM

POSITION SUMMARY

The Navigator is responsible for providing administrative and clerical support for the Community Hospice Palliative Care Team.

MAJOR AREAS OF RESPONSIBILITY

  • Communicates with both internal and external stake holders as to Community Hospice Palliative Care Team activities.
  • Responds to all general enquiries in a timely manner.
  • Ensure proper technical support and troubleshoots when necessary.
  • Manages all data relating to the program.
  • Assures confidentiality of the program.
  • Coordinates all the logistics related to service operations.
  • Controls all inventory for the Community Hospice Palliative Care Team.
  • Performs other job-related responsibilities and duties as assigned.

POSITION REQUIREMENTS

Necessary:

  • Clear criminal record.
  • Fluency in both (oral and written) French & English languages.

 Preferred:

  • Successful completion of a business administration or other related diploma or degree from a college or university.
  • Experience in hospice palliative care.

 Experience / Skills:

  • Minimum of 3 years of experience in an administrative function
  • Competency in Microsoft applications including Word, Excel, Outlook and Adobe
  • Keyboarding skills of 40+ wpm.
  • Excellent verbal and written communication and interpersonal skills.
  • Strong organizational skills with attention to details.
  • Ability to multi-task and to work well in a fast-paced environment.

CLINICAL MANAGER – RESIDENTIAL PROGRAM, Sudbury ON

The RPN assumes the care of those residents who are stable and have predictable outcomes and assists the Registered Nurse (RN) with the care of those residents who are unstable and have unpredictable outcomes. DESCRIPTION SOMMAIRE DU POSTE

L’IAA fournit des soins infirmiers directs aux résidents conformément aux politiques et procédures établies par MMH, notamment : l’évaluation et l’anticipation de l’ensemble des besoins physiques, émotionnels, spirituels et psychosociaux ; planification, documentation et mise en œuvre des plans de soins ; et évaluer l’efficacité des plans de soins et faire des recommandations de changements, le cas échéant. L’IAA prend en charge les soins des résidents qui sont stables et dont les résultats sont prévisibles, et aide l’infirmier(ère) autorisé(e) (IA) à prendre soin des résidents qui sont instables et dont les résultats sont imprévisibles. On s’attend à ce que les IPA soient des modèles positifs pour le personnel, les bénévoles, les invités, les membres de famille, les professionnels de la santé et la communauté.

 QUALIFICATIONS REQUISES

Obligatoires :

  • Permis d’exercice actuel auprès de l’Ordre des infirmières et infirmiers de l’Ontario
  • Certifié en “Learning Essential Approaches to Palliative” (LEAP Core) dans les 3 mois suivant l’embauche
  • Vérification du casier judiciaire, secteur vulnérable
  • Compréhension démontrée de la gestion de la douleur et des symptômes
  • Certifié en administration des médicaments

Préférées :

  • Expérience médicale / soins palliatifs récente, un atout
  • Cours – “Fundamentals of Hospice Palliative Care”, un atout
  • Bilingue (anglais / français), écrit et verbal, est un atout
  • Certifié en administration VI est un atout

Autres compétences :

  • Connaissance et engagement envers la philosophie des soins palliatifs
  • Capacité à communiquer efficacement, verbalement et par écrit
  • Capacité à organiser et prioriser le travail
  • Capacité physique d’exécuter les tâches du poste
  • Compétences développées en résolution de problèmes et en gestion de conflits.
  • Aptitudes interpersonnelles bien développées et capacité de travailler avec un résident/famille vivant avec et mourant d’une maladie avancée.
  • Compétences informatiques intermédiaires (Word, Excel et Outlook).

 SALAIRE : Échelle de 31,30 de l’heure

RÉGIME DE RETRAITE : Les employés occasionnels peuvent participer au Régime de retraite à prestations déterminées HOOPP

REGISTERED NURSE (RN)

Maison McCulloch Hospice (MMH) is a Centre of Excellence dedicated to the collaborative delivery of quality hospice palliative care in both official languages. We are growing and looking to hire interested candidates who want to pursue their career in a wonderful community.

POSITION SUMMARY

The RN provides leadership and direct nursing care to residents in accordance with established policies and procedures of MMH including: assessing and anticipating the total physical, emotional, spiritual, and psycho-social needs; planning, documenting, and implementing care plans; evaluating effectiveness of care plans and making recommendations for changes where necessary; and ensuring medication control regulations are adhered to. RNs are expected to be positive role models for staff, volunteers, guests, family members, health care professionals, and the community.

POSITION REQUIREMENTS

Necessary:

  • Bachelor of Science in Nursing
  • Certified in Hospice Palliative Care Nursing (Canada) [CHPCN(C)] or willingness to pursue CHPCN(C) certification with the Canadian Nurses Association (CNA)
  • Current license with the College of Nurses of Ontario
  • Certified in Learning Essentials Approaches to Palliative Care (LEAP Core) or willing to obtain within 3 months of hire
  • Police Vulnerable Sector Check
  • Demonstrated expertise with pain and symptom management

Preferred:

  • Recent medical / Hospice Palliative Care (HPC) experience
  • Bilingualism (English/French), written and verbal, (or willingness to pursue) is an asset

Other Skills / Abilities:

  • Knowledgeable about, and committed to, the Hospice philosophy of care
  • Ability to communicate effectively, both verbally and in writing
  • Ability to organize and prioritize work
  • Physical ability to perform the duties of the position
  • Highly developed problem-solving and conflict management skills
  • Well-developed interpersonal skills and ability to work with residents/families living with, and dying from, advanced illness
  • Intermediate computer skills (Word, Excel, and Outlook)

SALARY: Starting at $36.54 per hour

PENSION PLAN: Casual employees are eligible to participate in the HOOPP Defined Benefit Pension Plan
