COMMUNITY HOSPICE PALLIATIVE CARE NURSE (RN)

Part-time Position (averaging 30 hours / 4 shifts bi-weekly)

Working alternate Monday & Thursday (8:30 am to 4:30 pm)

& every second weekend (Saturday & Sunday 8:30 am to 4:30 pm)

POSITION SUMMARY

As part of the Hospice Palliative Care Team (HPCT), the Registered Nurse is responsible for providing telephone support / consultation to clients that have a life limiting illness and are in the last year of life, and their caregivers.

MAJOR AREAS OF RESPONSIBILITY

  • Provides comprehensive telephone assessments with a holistic approach, to address pain and symptoms experienced during multiple illness trajectories of life limiting illness
  • Performs assessments, care plan development and ongoing monitoring of each client in accordance with the established Maison McCulloch Hospice (MMH) Policies and Procedures
  • Reports all findings to the assigned Most Responsible Provider (MRP) whether this is the Physician or Nurse Practitioner
  • Schedules home visits for these providers so that client’s ongoing needs are met
  • Collaborates with the North East Cancer Centre, “Ontario Health atHome” and their contracted nursing providers

POSITION REQUIREMENTS

Necessary:

  • BScN in Nursing with a certificate of competence with the College of Nurses of Ontario
  • Minimum of 5 years full-time equivalent nursing experience
  • Ability to communicate effectively with clients and family in person or via telephone and troubleshoot any of their concerns
  • Receptive to ongoing education in Hospice Palliative Care
  • Must have valid driver’s license and access to a vehicle
  • Clear Vulnerable Person Check

 Preferred:

  • Recent minimum of 2 years full-time equivalent in Hospice Palliative Care (HPC) experience
  • Certified or receptive toward achieving certification in Hospice Palliative Care with the Canadian Nurses Association (CHPCN(C) designation)
  • Proficiency in French, (verbal and written)
  • Experience providing telephone nursing in a sensitive and compassionate manner
  • Ability to develop a plan of care with clients, primary caregivers and community providers
  • Strong knowledge of community resources and ability to refer as needed for various community services

 Personal Skills:

  • Demonstrates at all times clinical autonomy
  • Working knowledge of computer software applications Microsoft Office, (Excel, Word, Outlook)
  • Self-directed with exceptional organizational and interpersonal skills
  • Professionalism in all work settings (community in home setting and Maison McCulloch Hospice)
  • Ability to maintain confidentiality when sharing and discussing personal client information with the inter-professional team

SALARY: From $36.54 per hour

  • Keyboarding skills of 40+ wpm.

EXECUTIVE ASSISTANT, SUDBURY ON

The Sudbury Hospice Foundation (SHF) strives to be a premier philanthropic organization, transparent and accountable, ensuring the financial stability of Maison McCulloch Hospice to fulfill its Mission and Vision, in both official languages.

The Executive Assistant (EA) plays a vital role to the hospice operation. Reporting to the Executive Director, the EA is a public-facing role requiring excellent public service skills with residents, clients, staff, volunteers and donors. This role provides administrative support to all operational departments in the house and manages board-level administrative requirements for both the Maison McCulloch Hospice and Sudbury Hospice Foundation boards.  POSITION SUMMARY

  • Execute administrative support required to all members, committees and general activities of the Board of Directors for both MMH and SHF. This includes, but is not limited to, the production of meeting minutes, agendas, meeting packages and set up of meeting invites both in person and virtual.
  • Greet, welcome, and direct in person and telephone traffic to the house as required to support the Receptionist.
  • Process walk-in donations as required and assist in the tracking and processing of all donations received in person, by mail or over the phone.
  • Provide orientation tours to new residents, families, inquiries, new staff and new volunteers.
  • Assist in the provision of mass communication as directed for public events or internal purposes.
  • Set up for cheque presentations with special interest groups, take photos and engage with representatives around hospice needs and appreciation for their support.
  • Provide administrative support to all departmental leaders and areas of the home as requested.
  • Maintain tracking lists and produce reports as needed.
  • Other duties as assigned

POSITION REQUIREMENTS

  • Completion of post secondary education
  • Minimum of 10 years experience working in an administrative support capacity
  • Minimum of 15 years experience working with the public
  • Bilingual – Ability to communicate effectively, both verbally and in writing in both English and French
  • Police Vulnerable Sector Check
  • Excellent knowledge of computer software and social media applications
  • Strong organizational skills with significant attention to detail

 Other Skills / Abilities:

  • Knowledgeable about Hospice Palliative Care
  • Excellent social media skills (Facebook, Instagram, Tik Tok etc.)

  • Certified in LEAP Core (Learning Essential Approaches to Palliative Care) or willingness to complete within 3 months of hire

Preferred:

  • Recent clinical experience in hospice/palliative care (HPC) is preferred
