Terms and Conditions of Volunteering

I certify that I meet the minimum age requirement of 18 years old. I understand that the volunteers are placed according to the needs of the Hospice, their interests, skills, suitability and not all applicants are accepted. I understand that if I am accepted as a volunteer, I will be expected to demonstrate Maison McCulloch Hospice's values, abide by all policies and procedures, and fulfill the commitment I have made. I understand that a Vulnerable Sector Police Check may be a requirement to volunteer with the Hospice at my own cost. I agree to participate and complete the Orientation process as well as any required role specific training relating to my volunteer assignment. I understand that Maison McCulloch Hospice reserves the right to dismiss a volunteer from their volunteer position, if, in the opinion of the Hospice, continuance of the volunteer could cause detriment. I understand that providing false and incomplete information on this application may disqualify me from volunteering or result in my dismissal. I understand that all the information I provide on this application will be kept in confidence. By acknowledging that I understand these terms and conditions I consent to my personal information being collected for the sole purpose as indicated. I acknowledge that my information will be used, safely stored, and protected by Maison McCulloch Hospice for the duration that it is needed and/or my engagement as a volunteer. I declare that the information provided on this form is true and complete to the best of my knowledge.
